While I appreciate good design ()

here are several examples of one of the biggest problems we had on Kindle. I'm having especially strong flashbacks for pages 6, 15, 16, 18, 19 (holy crap!), 23, 24 (way to break conventions just for the sake of breaking conventions!), 25, 29, and 30.

Also, I'm sure 10 would have probably broken things in an interesting yet subtle way.

(One might say that seeing this gave me a TOC-tic.)

Funny, I have several of those books, including No. 10.

#10 wouldn't have any specific challenges for Topaz conversion, but there were some nasty ad-hoc things in the process due to handwavey spec things which various things relied on in not-very-well-structured manners. So, for example, I could see someone marking that page as a "toc" page which would then get special handling by the later processes, except there'd be no toc entries on the page to process.
